Prequalification Procedures and Instructions

To be considered as a contractor on the Owensboro Medical Health System Hospital Building Program, please complete the form below. Before you can be considered for contractor selection, the Request for Qualifications questionnaire and all required documents noted on the cover of the questionnaire must be completed and returned to us. These forms are located at subprequal.tcco.com.

Please be aware that even if you worked with Owensboro Medical Health Center, HGA, Smith Seckman Reid, and/or Turner Construction prior to this project you still must provide this first step package.  However, if you have already provided this package to Turner within the last 12 months, then there is no need to re-submit.
